The Nurturing Parenting Program and the Matrix Outcomes Model were chosen and designed to interact with each other in a way that optimizes participating families success. The Family and Community Support and Partnership initiative, funded by First 5 San Bernardino, couples these two services in a way that helps families achieve economic stability, safety, health and wellness while addressing parental attitudes and practice that are associated with risk of child-abuse and neglect. There are also many opportunities for parents to obtain support and stability through case management. Models range from online curricula to weekly meetings either in group or one-on-one, in-home settings.

So what are some of the basic steps that can done to ensure that isolation of trans people are limited. In fact, 41% of trans people have attempted suicide, while 57% have reported that family members have stopped speaking to them, and 60% have reported that a doctor has refused services. Furthermore, non-healthcare related services that transgender individuals need are often similar to others, such as, day-to-day activities, and yet stigma and stereotypes can often limit their ability to obtain basic services. Transgendered individuals face a plethora of barriers to care, often founded upon inadequate pronoun usage, access to care, and continuum of care stemming from stigma. Another major issue of identifying the population is that those in post-transition may no longer identify as transgender, though their needs remain higher than those who identify as cis-gender. Most population estimates are based on only those who are seeking services at gender clinics or those who receive GID diagnosis thus not truly reflecting the actual population. In the Inland Empire there are an estimated 27,000 transgendered individuals though this number is highly underreported.
